Documentation Policy

What’s in the document?

Regulated health professionals follow common standards for their competence and conduct. In our series, “What’s in the Document?” we will share information on key requirements for Health Care Aides (HCAs) in Alberta as established in documents from the College of LPNs and HCAs of Alberta (CLHA).

What Is the Documentation Policy?

CLHA policies outline requirements and expected behaviours for HCAs.

The Documentation policy describes the professional expectations of HCAs in producing clear, accurate, and detailed documentation. This document outlines the importance of effective, accurate, and timely documentation, potential legal problems, and risks that may result from poor documentation.

What’s in the Document?

The Documentation policy covers three major areas.

Professional expectations: HCAs are expected to have the knowledge and ability to document care provided according to legislation, regulatory documents, and employer requirements.

Principles of documentation: HCAs are expected to follow documentation principles for objectivity, accuracy, security, timeliness, and other best practices described in this policy.

Risks of poor documentation: Documentation provides a record of the care provided, and poor documentation can lead to errors, repeated or missing tasks, and poor client care.
