Reinstatement

(Previously Registered in Alberta)

This page describes the process for applicants who were formerly registered with the CLHA.

If this does not describe you, please take our applicant quiz.

To apply for reinstatement, you will need to have previously obtained registration as an Alberta LPN and meet the requirements of the CLHA’s Policy: Actively Engaged Requirements for Registration. See the Declarations and Registration Requirements Policy for more information.

If you do not meet the requirements, please contact the CLHA at lpnregistration@clha.com.

How to Apply

  1. Login to your MyCLHA account. Your username is the email address that we last had on file for you.
    • If you need to update your information, please contact our office through the Registration Department, Ask CLHA, or by phone 780-484-8886, or 1-800-661-5877 (toll free in Alberta).
    • If you need to change your email address, please call 780-484-8886, or 1-800-661-5877 (toll free in Alberta).
    • If you cannot remember your password, select “Forgot my Password.”
  2. Once logged in, select “Reinstate” on your dashboard to begin the Reinstatement application.
  3. Once you have filled out both application pages, you will be able to submit your application and pay the application fee.
  4. If your paid application has any missing requirements, the CLHA will contact you within five business days.
  5. You will receive a decision letter and final invoice for your practice permit fee within ten business days of all application requirements being met. You will need to meet all application requirements before you receive a decision letter.

We encourage all applicants to check their requirements through their MyCLHA account regularly. Monitoring your application status is important. Please remember, it is your responsibility to ensure that all necessary documents are submitted to the CLHA.

We are currently experiencing high call volumes. While we will strive to meet our service standard timelines, there may be some unavoidable delays.
