This page describes the process for applicants who were formerly registered with the CLHA.
If this does not describe you, please take our applicant quiz.
To apply for reinstatement, you will need to have previously obtained registration as an Alberta LPN and meet the requirements of the CLHA’s Policy: Actively Engaged Requirements for Registration. See the Registration Requirements Policy for more information.
If you do not meet the requirements, please contact the CLHA at lpnregistration@clha.com.
We encourage all applicants to check their requirements through their MyCLHA account regularly. Monitoring your application status is important. Please remember, it is your responsibility to ensure that all necessary documents are submitted to the CLHA.
Applicants who do not meet the CLHA’s requirements for reinstatement may need to complete additional education before returning to Licensed Practical Nurse (LPN) practice.
A practical nurse refresher program is available through Lethbridge Polytechnic. As part of the application process, applicants must complete the Verification for LPN Refresher Program form below.
The CLHA will review the request and provide the required verification directly to Lethbridge Polytechnic.
We are currently experiencing high email volumes. While we will strive to meet our service standard timelines, there may be some unavoidable delays.
Applicants and registrants can expect these standards when interacting with the Registration Department at the CLHA.
The Registration Service Standards policy outlines registration service standards at the CLHA.
